Managing Patient Addresses

Accurate address information is essential for effective communication, billing, emergency contact, and in certain cases, home-based care. The Address section in the Patient Information module ensures that the care team always has the latest and most reliable contact details, reducing errors and improving patient safety.


How to Access and Use the Address Section

1. Navigating to Address Information

  • Open the Patients tab and select the patient profile.

  • Click the Patient Information tile.

  • From the sidebar menu, select Address to view all recorded addresses.


2. Viewing Existing Addresses

  • All saved addresses appear in a list, labeled by type (e.g., Home, Postal, Physical).

  • Each entry displays the full address for quick review.


3. Adding a New Address

  • Click Add Address at the bottom right.

  • Fill out the form fields (see below).

  • Click Save to add the address to the patient record.


4. Editing or Deleting an Address

  • From the address list, click the three-dot (⋮) menu next to the entry.

  • Choose Edit to update details or Delete to remove the address.


5. Address Use Types

You can record multiple addresses for a patient, each serving a different purpose:

  • Home: The patient’s primary residential address.

  • Postal: Mailing or correspondence address.

  • Physical: An alternate location, such as a work or secondary residence.

Select the appropriate type from the Use dropdown when creating or editing an address.


Fields to Complete When Adding an Address

Field

Description

Address Line 1

Street address (e.g., number, street name).

Address Line 2

Additional details: apartment, suite, unit, building, etc. (optional).

Use

Choose the address type: Home, Postal, or Physical.

City

Enter the city or locality.

State

Select the state or province from the dropdown.

ZIP Code

Postal or ZIP code.

Tip: Use the “Use” field to classify addresses clearly, especially when patients have multiple records.
