Patient Contacts: Adding and Managing Key Relationships

Keeping the Contacts section updated ensures providers can quickly reach the right people for emergencies, care planning, or administrative needs. Here you can add, view, edit, and delete a patient’s important contacts—such as family members, emergency contacts, employers, or insurance representatives.


How to Access and Use the Contacts Section

Accessing Contacts

  1. Go to Patients in the top navigation.

  2. Open the patient’s profile.

  3. In the sidebar under Patient Information, select Contacts.


Adding a New Contact

  1. Click + Add Contact.

  2. Fill in the fields in the Contact form:

    • First Name, Last Name

    • Biological Sex (Female / Male)

    • Relationship (Emergency Contact, Employer, Next-of-Kin, etc.)

    • Email and Phone

    • Address (street, city, state, ZIP, and type: home, postal, or physical)

  3. Click Save.


Viewing Contacts

  • Once added, contacts will appear in a list.

  • Each entry displays Name, Relationship, Email, Phone, and Gender.


Editing a Contact

  1. Locate the contact in the list.

  2. Click the three-dot menu (⋮) next to the contact.

  3. Select Edit, update details, and click Save.


Deleting a Contact

  1. Locate the contact in the list.

  2. Click the three-dot menu (⋮).

  3. Select Delete and confirm.
