Lab Requisitions

Learn how to retrieve lab requisitions on your orders.

The Lab Requisition is important for your patient to provide to the PSC (Patient Service Center) when they go to get their blood drawn or including it with the blood when doing an in-office blood draw. This requisition is what links the order to your patient and will facilitate the delivery of results back into Charted Health.

Important Order Statuses

  • Draft - Orders in this status have not yet been paid for

  • Requisition Pending - Payment has been received and transmitted to the lab, awaiting a requisition.

  • Requisition Available - Electronic requisition has been received.

  • Preliminary Results - the laboratory has begun transmitting results for the corresponding order.

  • Final Results Available - All results have been received for the corresponding order.

Viewing a Lab Requisition

The lab order originates within a patient record. To access the lab requisition for a particular patient, you will want to:

  1. Navigate to a patient record. You can access your list of patients by clicking on "Patients" in your top navigation bar. From here you can scroll through your patients or type their name in the box to search by name.

  2. Select the "Labs" application by clicking on the icon.

  3. Identify the order's requisition that you are trying to access, you will want to click on the three dots that can be found to the right of each lab order.

  4. Select "View Requisition" and the PDF will be opened in a new tab within your browser.

Note: Lab Requisitions are also available for a patient within the Patient Portal. Patients will follow the same steps as above from the "Labs" section.
